VPK Information for Providers
PROVIDER REGISTRATION

Thank you for your interest in Florida’s Voluntary Prekindergarten Education Program (VPK). Please follow these steps to register for the program.

  1. In order for you to receive payment for the VPK program, you must have completed and submitted both forms 10 and 11 of the Voluntary Pre Kindergarten Application.

    Form AWI-VPK 10 (Statewide Provider Registration Application). This form provides basic contact, licensing, and accreditation information about each provider or school.

    Form AWI-VPK 11(Class Registration Application). This form provides more detailed information about each provider’s or school’s VPK classes, curriculum, instructors, and class schedules.

    Use this Application Checklist to ensure that you have all the documents needed to process your application to participate in the Voluntary Prekindergarten Program

  2. Once your application has been approved by a Coordinated Child Care representative, you will then need to submit two copies of the Provider Agreement Forms. Note: these no longer need to be notarized

    You may mail the signed and notarized forms to

    or you may personally deliver them to the same address.

  3. Once CCC has received your Agreements they will be signed by the Early Learning Coalition of Pinellas, Inc. and a copy will be mailed back to you for your records.

  4. Please fill out this VPK Enrollment Form for all new students that you have in this program.

  5. Create your VPK calendar. If you need help calculating your days and hours you can use one of these Microsoft Office Excel files Summer Calendar 08 or School Year Calendar 08-09 double click on the files and choose save

Documenting and Certifying Student Attendance in the VPK program


VPK Changes

The VPK Change Request Form is used to report changes within an established VPK program to Coordinated Child Care.

Instructions for Completing the VPK Change Request Form:

VPK Teacher Changes:

Please submit the following information when requesting a change in VPK teacher(s):

Background Screening:

  • FBI Letter
  • FDLE Report
  • Local Screening
  • Affidavit of Good Moral Character
  • Verification of Work History

Educational Credentials:

  • Lead Teacher:
    • CDA Certificate or Diploma for AA, AS, BA, BS
  • Assistant Teacher:
    • High School Diploma/GED or higher

5.0-Hour Literacy Training:

  • Required for Lead teachers who meet requirements with a CDA only


Schedule Changes:

  • If you are reporting changes to your VPK schedule the total number of hours for a school year session must equal 540 and 300 for a summer session.

  • School year sessions must end on or before the last day of the Pinellas County School year and summer sessions must end before the start of the Pinellas County School year.

Director/Owner Changes:
If the previous Director or Owner signed the VPK agreement, a new one is required.

Other:
Use this section to report any changes to your VPK program hat do not fit into the above categories.
